Error when Using Caché Studio Hooks: <UNDEFINED>PreST+15^%cmtP2

Symptoms
If the VC/m Cache Studio hooks are installed, when trying to save a routine, the error <UNDEFINED>PreST+15^%cmtP2 is given, and the routine is not saved.
Cause
This error is due to an InterSystems bug in the Caché Studio Hooks. It is known to occur when the server is running Caché 4.0.3 (Build 151) and 4.0.4. It is known to be fixed in 4.1.1 (Build 178).
Solution
Contact InterSystems support, refer to devchange MAK139 and ask for a Objects Patch which includes this fix.
